Discharge management

Our discharge management team is responsible for ensuring a seamless transition after treatment. It is important to arrange necessary appointments while you are still in the clinic in order to avoid long treatment gaps and waiting times.

In consultation with the doctors and therapists involved, we will determine which medical or nursing measures are necessary following your hospital stay in order to ensure that your treatment is as successful as possible. This also gives you the assurance that, for example, any necessary medication will be available to you seamlessly or that you will be able to attend any necessary treatment appointments immediately after your return. In the “parent company” of the Rheinclinic, the VITANUM, an experienced medical team and a state-of-the-art pharmacy are also at your disposal.
